The items listed here are items that are either necessary, or helpful in completing our projects for the families. Hopefully, having everything here will make it easier for you to download what you need, when you need it.

Should there be an item that is missing or appears to be outdated, please e-mail headquarters@nilmdts.org.  We update these forms regularly.

Parent's Consent Form - English This is a Mandatory Form for all Sessions. Version 10-2009.

Hospital Q&A - Document for medical professionals outlining how NILMDTS works in hospitals

Hospital Release Form - Optional document to be used/modified by hospitals prior to making requests with NILMDTS photogs on the patient's behalf

Hospital Release Form - Spanish - Spanish version of the document to be used/modified by hospitals prior to making requests with NILMDTS photogs on the patient's behalf

Letter to Parents - Document to send to a family a couple days ahead of mailing the actual CD/DVD of images.

Letter to Parents-Spanish - Document to send to families.  This references both a CD/DVD so if you are not sending a DVD you will need to customize the letter.

Info Sheet - Includes latest NILMDTS statistics, etc 

25-Week Gestation Guideline - Details on why NILMDTS criteria is that babies must be at least "25-weeks gestation" or at the discretion of medical personnel to be considered candidates for the program

Fundraising Policy - Strict rules which all volunteers must abide by regarding fundraising for NILMDTS

Display Reservation Application - All NILMDTS Active Volunteers must fill this form out in order to request a reservation for a display

Media Guidelines - For NILMDTS Photographers - includes info on how interviews work, how to request assistance from NILMDTS public relations team, sample questions from the media, etc.

Generic NILMDTS Press Release Template for Photographers - A letter to send to a member of the press and a press release/info sheet

Photographer's Recruiting Video - This is available to our members only on our box.net account.  This link will take you to the box.net website, but you must have our login information to access.  Look for the folder titled HQ * DO NOT DELETE *.

Incident Report Form - This form needs to be filled in, and submitted to us, to report situations that may be outside of our normal operating procedures

PPA Merit Application - This should be filled out by PPA members who have attended a NILMDTS training in order to receive a PPA merit.
