Volunteer of the Year Award Nomination Form

The Volunteer of the Year Awards are an opportunity for Now I Lay Me Down to Sleep to recognize and celebrate volunteers who have gone above and beyond the call of service in pursuit of serving our families. Awards will be given to one person in each volunteer position at NILMDTS: Affiliated Photographer, Digital Retouch Artist, Community Volunteer, Assistant to Photographer, Dispatcher, Walk Volunteer, and Area Coordinator.

Eligibility:  Any currently active volunteer in good standing can be nominated for this award.

Award Criteria: Nominees will be evaluated based on the following criteria:

  1. Dedication to NILMDTS mission and record of positive impact on NILMDTS families or fellow volunteers
  2. Length of service with NILMDTS
  3. Number of sessions participated in (if applicable for volunteer role)
  4. Commitment to NILMDTS’ Core Values:
  • Passionate Contribution: We believe that collectively we make a powerful impact on the healing journey of families. We contribute to our role in the organization in a way that demonstrates passion and commitment.
  • Prioritize People: We take pride in having a positive mindset about our team and consider the perspective of others. We show appreciation to each other. We encourage each other to rest and rejuvenate, prioritizing our health and most important relationships, so we can make our greatest contribution.
  • Unyielding Integrity: We are honest, do what is right, and honor our commitments. When we make a mistake, we own it, resolve it, and learn the lesson to avoid it in the future. When someone deserves credit and appreciation, we acknowledge their efforts and achievements.
  • Continuous Growth: We constantly strive to improve the mission of the organization as well as improve ourselves. We are open to improvement, growth, and change so we can better serve more families.

Nomination Information: Please include a description of no more than 500 words describing your nominee and why they deserve to be named Volunteer of the Year for their category. Please refer to the criteria as applicable in your nomination, and be sure to reference at least one NILMDTS Core Value. Additional information in this narrative can include: the volunteer’s history with NILMDTS, their responsibilities and roles, the impact they’ve had on fellow volunteers or families, and why you nominated them. Self-nominations will not be accepted. You may nominate more than one volunteer for each category.

Nomination Deadline: Nominations must be submitted by end of day Monday, October 31, 2022.

Recipient Selection and Notification: Nominees will be voted on by members of the NILMDTS Board of Directors. Recipients will be celebrated at the end-of-the-year virtual volunteer celebration on December 6, 2022 at 6pm MT.
